A) You communicate with the psychologist via the website’s contact form or via an email message in order to book an appointment, in which case you will be asked to provide your personal data, specifically your full name, email address, and phone number. The information you fill in the contact form is sent directly as an email to the psychologist and is not recorded in any third-party database.
In the above case, filling out and sending the relevant contact form or email message to the psychologist means that you explicitly, freely, and with full awareness state that you consent and agree to the collection of your relevant personal data by the psychologist.

The psychologist does not share your personal data with third parties. Exceptionally, she transfers to the professional providing accounting services only the personal data that is absolutely necessary for the fulfillment of her tax obligations (issuance of tax invoices), meaning your full name and address. Further data requested in the website’s contact form and potentially in the course of providing the psychologist’s services are protected by professional confidentiality.

6. Right of Access, Rectification, and Erasure of your personal data
You have the right to access your personal data processed by the psychologist, as well as the right to request the rectification of any inaccurate data concerning you (Art. 15 of Regulation (EU) 2016/679). Taking into account the purposes of the processing carried out by the psychologist within the framework of your contractual relationship, you may also require the completion of incomplete personal data, including by means of a supplementary statement (Art. 16 of Regulation (EU) 2016/679).
Right to erasure (“right to be forgotten”): You have the right at any time and at your absolute discretion to request from the psychologist (who, as previously stated, acts as the Data Controller in your contractual relationship) the erasure of your personal data in accordance with the specific provisions of Art. 17 of Regulation (EU) 2016/679. Your data is stored for as long as necessary for the psychologist to be in full compliance with her legal obligations (Art. 23 of Regulation (EU) 2016/679).
